Field Trip Medication & Allergy Forms

Our Medication and Allergy Forms were updated last year (February 2025). Please use the updated versions for all field trips.

These forms are only required for students with allergies or those who need any type of medication (prescription or over-the-counter such as Tums, Tylenol, etc.). 

Important Reminders:

• One form per medication is required, with both doctor and parent signatures—even for over-the-counter medications.
• Travel-size medications are encouraged to help lighten field trip backpacks.
• RHLS no longer provides office-supplied OTC medications. If a student may need them, parents must supply both the form and the medication.
• Forms should be submitted at least 2 weeks before the field trip.
• Make sure to keep your forms updated.
